Notification of Claimed Copyright Infringement
If an infringement has occurred a copyright holder may send a notification of claimed infringement to the designated agent below. This must include the following (512(c)(3)(A)(i-vi)):
A physical or electronic signature of a person authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
Identification of the copyrighted work claimed to have been infringed, or, if multiple copyrighted works at a single online site are covered by a single notification, a representative list of such works at that site.
Identification of the material that is claimed to be infringing or to be the subject of infringing activity and that is to be removed or access to which is to be disabled, and information reasonably sufficient to permit the service provider to locate the material.
Information reasonably sufficient to permit the service provider to contact the complaining party, such as an address, telephone number, and, if available, an electronic mail address at which the complaining party may be contacted.
A statement that the complaining party has a good faith belief that use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law.
A statement that the information in the notification is accurate, and under penalty of perjury, that the complaining party is authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.

Counter notification:
An alleged Copyright infringer may file a counter notification to the FaithStories.org. The counter notification must include (512(g)(3)(A-D)):
A physical or electronic signature of the alleged infringer.
Identification of the material that has been removed or to which access has been disabled and the location before removal.
A statement under penalty of perjury that the alleged infringer has a good faith belief that the material was removed or disabled as a result of mistake or misidentification of the material to be removed or disabled.
Their name, address, and telephone number, and a statement that they consent to the jurisdiction of Federal District Court for the judicial district in which the address is located, or if the subscriber’s address is outside of the United States, for any judicial district in which the service provider may be found, and that they will accept service of process from the person who provided notification under subsection (c)(1)(C) or an agent of such person.

If we receive a notice that content infringes another party’s copyright, the member’s account may be terminated and the content will be removed from FaithStories.org. What are you doing to prevent copyrighted content from appearing in FaithStories.org?
Since all content on FaithStories.org is posted by our members, we make it clear in our terms of use and at the time of upload that they must own the copyright to the videos they post or have permission from the copyright holder. We take copyright issues very seriously and encourage copyright holders to contact us if they have any objections about specific videosówe will cooperate with them to take down any infringing content immediately. Repeat and flagrant offenders of the terms of use are addressed with proprietary technology.
How do I report copyright infringement?
If you believe that someone else has uploaded your copyrighted content without your permission, please contact us to submit a copyright notification.
What if I have been falsely accused of copyright infringement?
We’ll let you know if we receive a copyright complaint about any of your video content that is hosted on FaithStories.org. We’ll also give you the opportunity to file a DMCA counter-notification.
How do I report inappropriate videos?
If you see a video that you think is inappropriate based on our terms of use, you can report it to us for review by clicking the “Report” link under the video.
How does FaithStories.org handle inappropriate content?
Our community understands the terms of use and effectively polices the site for inappropriate content (similar to other open internet communities such as eBay and craigslist). Their vigilance combined with our proprietary technology helps us to enforce our community standards.
